You can't chalk it down to one reason. It's a combination of several different factors. It could be that they come from a family that puts emphasis on external appearance. It may be that they're around people who're placing emphasis on appearance. As well (s)he might be constantly made fun of about the way (s)he looks. Additionally, the media keeps reinforcing a size zero. So it all comes down to nature versus nurture and in my opinion it's both. It's a combination of all these factors I think.
